Requirements for Food Bags: A valid and
readable picture ID along with proof of residence: Name must match on ID and
proof of residence document. Proof of Residence document accepted: Utility,
Water or Phone Bill (Dated Nov. or Dec. 09); A rental lease (no older than 2
years. (Apartment addresses must include an apartment number on the ID and
bill); A current Housing Authority Rental Statement or Mortgage Deed; or a SSI
Awards Letter (no older than 90 days).
WE DO NOT ACCEPT THE FOLLOWING as proof of
residence: P.O. Box #, Social Security Card, Voter Registration Card, Court
documents, or Medical or Credit Card bills. IF Turkeys are available, you Must
have proof of residence along with a valid ID to receive a turkey and Only One
Turkey will be issued per house-hold. Persons living in rented rooming property
must provide a notarized letter from landlord which must include landlord's
contact information which will be verified. |
